Patient Instructions

- Photo ID (driver's license or government-issued ID)
- Insurance cards (Medicare, Medicaid, or commercial)
- List of current medications and dosages
- A responsible adult to drive you home after procedures if conscious sedation is used
- Arrive 30 minutes before your scheduled time
- Wear comfortable, loose-fitting clothing
- Continue your dialysis schedule as instructed by your team
- Inform staff of any allergies, especially to contrast dye or iodine
- Take your regular medications with a small sip of water unless told otherwise
- Anticoagulants (warfarin, Xarelto, Eliquis): discontinuation is generally not required prior to your procedure — follow your physician's specific instructions
- Diabetic medications: follow your physician's guidance on procedure day dosing
- Contact us with any questions about medication adjustments
- Most patients leave with just a band-aid and resume normal activities
- Keep the access site clean and dry for 24 hours
- Watch for signs of bleeding, swelling, or infection
- Follow your dialysis center's recommendations for next session timing
If you have a latex allergy, please notify our staff immediately upon arrival. If you experience an emergency, call 911 or go to your nearest emergency room. For urgent access concerns, you can contact us or your dialysis unit.
Admissions
We make the admissions process as straightforward as possible so you can focus on your health.
To schedule an appointment, call us at (386) 274-4244 or complete the appointment request form on our main website. We accept referrals from nephrologists, dialysis centers, and primary care physicians. Self-referrals will require a referral and authorization from a healthcare provider.
Upon arrival, you will be asked to complete registration forms and provide your insurance information. Our staff will verify your insurance coverage and obtain any required authorizations prior to your procedure.

- Prior authorization is obtained by our team when required
- Spanish-language assistance and interpretation services are available
- Patients with mobility limitations should call ahead so we can prepare appropriate accommodations

Daytona Beach Vascular Access participates with Medicare and most major insurance plans. Your out-of-pocket costs will depend on your specific insurance plan, deductible, and co-insurance requirements. About Service Bundle Estimates: The service bundle information provided on the AHCA pricing website is a non-personalized estimate of costs that may be incurred by the patient for anticipated services. Actual costs will be based on the specific services actually provided to the patient on the date of service.
Your Right to a Personalized Estimate: You have the right to request a personalized estimate of costs from Daytona Beach Vascular Access before your procedure. To request a personalized estimate, please contact our office at (386) 274-4244 or office@daytonavascular.com.
Financial Assistance, Charity Care & Collections: Daytona Beach Vascular Access does not currently operate a formal charity care program; however, patients experiencing financial hardship are encouraged to contact our billing department to discuss payment plan options and available assistance. Unpaid balances may be subject to standard collection procedures after reasonable notice to the patient. Please contact our office to discuss your individual situation.

Billing & Regulatory Information
Disclosures: the Vascular Center is a facility in which physician(s) have an ownership or investment interest. The list of physician owners or investors is available upon request. This facility is fully licensed by the State of Florida, is Medicare certified, and holds Joint Commission Accreditation as an Ambulatory Surgery Center (Gold Seal of Approval™, ID: 716895). - Services may be provided by the Vascular Center as well as by other health care providers who may separately bill you and who may or may not participate with the same health insurers as the Vascular Center
- You have the right to request an itemized statement of charges for services rendered
Frequently Asked Questions
Office visits, vessel mapping, and access ultrasound evaluations: typically a 30-minute evaluation.
Catheter procedures (placement, exchange, or removal of tunneled hemodialysis catheters): typically 30–60 minutes.
Dialysis access procedures (fistulagram, angioplasty, stent placement): typically 1 hour.
Thrombectomy procedures (mechanical declotting of a fistula or graft): may take 1–2 hours depending on complexity.

the Vascular Center provides free language assistance services to patients with limited English proficiency. Interpreter services are available for your appointment — please notify us in advance so we can make arrangements.
